From time to time, the status of a provider may change, new providers may be added and others may leave the network or decline to accept new patients. If your plan uses our national dental network, the CAREINGTON PPO dental network or the EyeMed Vision network and your provider is not a member of that network, you can nominate your provider for membership.
Simply click the appropriate nomination form, print and complete it, and then submit it per instructions indicated on the form. Your provider will be invited to participate in the network.
